Applications will be accepted beginning July 1, 2026. Applications are available online at lcohc.com or may be picked up at the LCO Health Center.
Reimbursement checks will be distributed every Friday from 2:00 p.m. to 4:30 p.m., beginning July 17, 2026. Checks may be picked up at the registration counter from Barb Lacapa.

If you would like someone else to pick up your check on your behalf, please notify us in advance by phone or provide a signed note authorizing that person to do so.
