Designated Check Pickup Policy

If an Elder is designating another individual to pick up their check, the designated person must bring a signed letter from the Elder.

The letter must include:

• The Elder’s full name
• The Elder’s enrollment number
• The name of the individual being granted permission to pick up the check.
